I've seen small 1.5-12v motors being reduced in speed by gearing mechanisms. Some have the mechanism on the outside of the motor, others have the mechanism concealed within the motor housing itself. Is there any difference or advantage ?

Interior gearing would be far less prone to damage from external factors such as dust or foreign objects. External gearing would be easier to lubricate but subject to contamination. Generally speaking external gearing is easier to take apart for gear ratio changes.
